Attention Vets! Print a Fall 09 Form D, complete, and submit it to the Veterans Office as soon as possible for certification.

If you are collecting benefits at UConn for the first time please attach a copy of your certificate of eligibility from the veterans administration.

The Form D must be signed by your advisor.
